Ophidic envenomation is a global public health issue due to its high morbidity and mortality rates in tropical countries. We report the case of a 49-year-old female patient with chronic malnutrition from a rural area who initially resorted to traditional medicine and spiritual practices following a snakebite. She was referred from a primary care center due to the risk of septic shock after necrosis was found in her right lower limb, along with signs of systemic inflammatory response. Additional tests and imaging were ordered, leading to the diagnosis of septic shock secondary to a severe Bothrops snakebite. Initial management included intravenous fluids, antibiotics, analgesics, and vasopressors. She required a transfemoral amputation and developed complications related to her clinical condition, necessitating admission to the intensive care unit for two days, which culminated in death secondary to multiorgan failure. This highlights the importance of seeking urgent medical attention after a snakebite in order to prevent adverse outcomes.
