During pregnancy, the immune response undergoes adjustments, tending mostly to preserve the new being in development, therefore, the hydatidiform mole coexisting with a live fetus is a rare entity that is characterized by hydropic degeneration of the chorionic villi and the trophoblast hyperplasia where there is gross evidence of a fetus. Its objective is to be able to show a clinical case of a twin pregnancy affected by a hydatidiform mole and a live fetus, through a search of a bibliographic review from a year ago, both in Spanish and English. The clinical case of a 28-year-old pregnant woman named Camila Pazmiño is presented, with a gestational age between 26 and 27 weeks, with an obstetric history of G2, P1, A0, who was admitted with a diagnosis of partial molar pregnancy and viable fetus, in addition She refers to a threat of immature labor where a normal delivery is performed, a low-weight newborn that does not survive due to immaturity and mola is extracted in its entirety, obtaining a female newborn, weighing 1000g, and the Apgar score. on 8/10 and transferred to the Neonatology service. Also to rule out that molar pregnancy with a live fetus is of high maternal and fetal risk, for which it is proposed to establish and identify early diagnosis and management through invasive techniques such as amniocentesis or chorionic villus biopsy, and thus be able to carry out follow-up. careful perinatal care, where the interruption of pregnancy must be individualized in each patient and avoid previous complications
