Tomographic considerations during the decision for endodontic retreatment or tooth extraction. A case report

Abstract

The aim of this report is to point out the tomographic considerations not observable in the periapical radiography that allow a correct treatment decision. Emphasis will also be placed on how to evaluate the level of difficulty in case of retreatment and possible long-term failure. A 64-year-old female patient came for consultation without any pain symptoms referring that she feels dissatisfied with the appearance of her tooth (OD# 1.6), which had been treated more than a year ago. The tomographic analysis confirmed certain difficulties such as calcifications that prevented complete visualization of the canal, steps and deviations from the normal anatomy of the palatal canal, considerable curvatures at the apical level and difficulties for the complete rehabilitation of the tooth due to the scarce remaining tooth structure at the coronal level. For this reason both patient and treating dentist opted for exodontia and placement of a dental implant
